Eligibility Criteria
Seniors: Must be 55 years or older.
Individuals with IDD: No age restrictions, but must have a diagnosed intellectual or developmental disability.
Veterans: Must be a former member of the armed forces with eligibility for veteran-related services
Must require assistance with daily living activities, such as bathing, dressing, medication management, meal preparation, or transportation.
Residents should not require 24/7 skilled nursing care but must need a supportive living environment.
For day habilitation (dayhab) services, individuals should benefit from daily structured activities to promote personal development.
Residents must provide medical records and documentation of their health status, including a current diagnosis and any relevant care plans.
Any potential resident should be free of communicable diseases that may pose a risk to others in the community.
Potential residents must undergo an initial behavioral and medical assessment to ensure our community can meet their specific care needs.
Individuals with severe behavioral challenges or conditions requiring specialized medical care may require additional evaluation to determine eligibility.
